This month we celebrate two location anniversaries. Our north Raleigh location at AIHF celebrates its 5 year anniversary and Holly Springs celebrates its one year anniversary. Carolina Family Practice & Sports Medicine was recently recognized by NCQA (National Committee For Quality Assurance) for achieving Level 3 Recogntition as a Patient-Centered Medical Home.
What does this mean for our patients?
Patient-centeredness has long been a central theme to how CFPSM providers and staff approach healthcare. By putting the patient at the center of his/her medical care, our goal is for our patients to take ownership of his/her own well-being.
In order to achieve this recognition, physicians & staff worked side by side to identify, improve, and document processes within our workflow.
With these parameters in place, we are better able to serve our patients on a synergistic level.
